Why did I receive a W-9 form?

U.S. Bank sends these notices when an account has an invalid Social Security number U.S. Bank sends these notices when an account has an invalid Social Security number (SSN) or Tax Identification number (TIN). We send these notices to meet Internal Revenue Service (IRS) requirements.

If you received this notice, it means we don’t have your SSN or TIN on file, or the number provided does not match IRS or Social Security Administration records.

This is an official notice, so please follow the instructions. If you don’t, it could result in additional tax withholding and possible issues with the IRS.

Here's a few other things you should know:

  • The notice comes with a W‑9 form and a return envelope addressed to U.S. Bank’s Legal Research department.
  • We ask that you fill out the form and include your SSN or TIN so we can make sure your account information is correct.
  • The W-9 form can also be filled out at any U.S. Bank branch.
