How do I request a replacement debit card?

Ordering a replacement debit card is easier than ever. If it's damaged, you can order another card with the same number. If it was lost or stolen, see: How do I report my debit card as lost or stolen?

To replace a debit card using online banking

  1. Select your checking account from the dashboard.
  2. Choose Account (above your available balance), then select Details & controls in the Card info tile (to the right of the Details).
  3. Locate the Control section and select Replacement card.
  4. Choose the card you want replaced and select Continue.
  5. Choose the reason for the replacement, then select Continue.
    • Lost, Stolen, or Never received: A new card number is issued.
    • New card design or Damaged: A new card with the same card number is issued. 
      • An image of your debit card will display.
      • Change to a new card design or keep the existing one, then choose Select this design.
  6. Review your address and the order details, if everything's correct, select Submit my order.

To replace a debit card using the U.S. Bank Mobile App

  1. Open the main menu, choose Manage cards, then select the debit card to replace.
  2. Select Order a new card within the "Control" section, choose the reason for the card replacement and select Next.
  3. An image of your debit card will display. Change to a new card design or keep the existing one, then select Next.
  4. Review the address and select Next.
  5. If your order details are correct, select Submit.
